Incident / accident form.
If an incident or accident happens during training, a match or any club activity, please use this form to record the details as soon as possible. It helps us make sure the right follow-up takes place and keeps our club records accurate and up to date.
When should I complete this form?
Please report any accident or incident that takes place during a Rothley Imps training session, match, tournament or other club activity.
This includes injuries requiring first aid, incidents where a player has to leave a session, situations where emergency services are contacted, or anything else that you feel the club should have a formal record of.
What information will I need?
Please provide as much detail as possible, including:
Where and when the incident happened
Who was involved and who was responsible for the session
What happened, including the activity taking place at the time
Any injury sustained
First aid or other action taken, including the name of the first aider
Whether a parent/guardian, ambulance or police were contacted
What happened afterwards — for example, whether the player continued, went home or attended hospital.
Please report it promptly
Complete the form as soon as reasonably possible after the incident, while the details are still fresh. Keep the information factual and avoid assumptions or opinions.
Safeguarding concern?
If an incident raises a concern about the welfare or safety of a child, it should also be raised promptly with the Club Welfare/Safeguarding Officer through the club’s report a concern page or directly with the CWO.
